Because there is a lot of confusion about this, I clear something up about this condition that I deal with in my book “Trapped in the Wrong Body”: The Boxer is a Man.
What happens is that we are facing a case of an athlete with a sexual developmental disorder, carrier of XY chromosomes, who has testicles (usually internal), levels of typical male testosterone and functional androgen receptors, but whose case is usually described as women with “hyperandrogenism”, that is, women with abnormally high testosterone levels but they are men.
These people experience the physical benefits of high testosterone during puberty, which translates into athletic advantages over women.
The problem for sport and competition criteria is that these athletes with a sexual developmental disorder related to 5-alpha reduced (5-ARD) deficiency can be socialized as women, can be legally women, and can live and identify as women; but they are men. Again: they are MEN.
These individuals are usually born with female-looking genitals, which can lead to them being misassigned as female sex.
5-ARD is caused by a mutation in the gene that codes the reduced 5-alpha enzyme, which turns testosterone into a more powerful androgen, DHT. This androgen interacts with the androgen receptor, such as testosterone, and is necessary for the typical development of the external male genitals (penis and scrotum) and the prostate. Without DHT, typical female external genitals develop.
JJOO organizers are aware that athletes with 5-ARD are male and are experiencing the benefits of male puberty.
The requirement to reduce your testosterone to typically female levels is not discriminatory, as it concerns males asking to compete in the female category. But more importantly, all the relevant scientific evidence proves that reducing male T in adulthood does not invalidate the physical benefits of male puberty.
